Registration Deadline: Final Registration is Wednesday, June 1, 2011. There will be no onsite registration!!!! Use Team Registration Form.

Age Division: All golfers welcome but must have a current USG, USGA, or Local club handicap established. Teams can be mixed. They may also include PGA proffesionals with a handicap of 0.

8:00 Putting Competition
9:00 AM Tournament begins
Rules: Format will be 4 person scramble in net handicap scoring. A team handicap will be given prior to tee-off. Teams may be mixed (male/female). Prizes for longest drive, closest to the hole and 5 places will be given for 15 entries (more if more teams participate). Prize amounts determined by total field entry. All prize money will be run through Cedar Ridge Golf Course as merchandize. A raffle will be held for additional prizes. A total of 120 players (30 teams) will be accepted on a first come basis. Competitors must have a current UGA, USGA, or Local established handicap or will be assigned a zero.
Players with a handicap 0-9 will play blue tees, handicap 10-20 will use white tees, female players will use red tees.
Confirmation: Internet Registrations will immediately receive a confirmation e-mail. Mailed or delivered registrations can check at registration area before event.
Check-In:
Soccer Teams: All teams must check in on either Tuesday, June 14 from 4:00 PM to 7:00 PM or, Wednesday, June 15, between Noon and 7:00 PM. at the SUU Sorenson PE Building, 560 W University Blvd., Cedar City. These will be the only opportunities for check in and teams must arrange for a representative to check the team in during these times. Teams will not be allowed to participate without first checking the team in. No additions to team rosters after final check-in are allowed. U-12 through U-19 teams are limited to a roster of 18 players and three coaches. U-9 through U11 teams are limited to a roster of 14 players and three coaches. Coaches, or an authorized adult team representative, MUST bring the following for check in:
Athletes and Teams (excluding soccer): need to check in at the SUU Sorenson PE Building, 560 W University Blvd., Cedar City. Each athlete will receive a welcome packet containing the official Utah Summer Games Athlete shirt. Teams should allow for an hour to check in so that they are not late for their first game. Registration will be open on the following days:

All teams must have waivers signed by each team members if they are 18 or older, and by parents/guardians if they are under 18. NO EXCEPTIONS.
Refund Policy: NO REFUNDS will be given, including for medical withdrawals. If an event cannot be held due to inclement weather a voucher code may be issued to be used for another sport during the 2011 event or the same sport during the 2012 Utah Summer Games. Exceptions will be made only for the following: entries received after entry deadline; sports or competitions reaching maximum registration capacity; entries received after the maximum number of teams are bracketed or if there are not enough teams or individuals to form a division. Requests must be in writing by email to hutcheson@suu.edu and submitted no later than July 1, 2011. All team voucher requests must be submitted prior to schedules being issued.
